Understanding Structural Ageing: Beyond the Surface

When we talk about ageing, the focus is often on wrinkles and skin laxity. However, the visible changes we associate with ageing are largely driven by deeper structural shifts — specifically collagen depletion, bone density loss, and changes in adipose (fat) compartments. Understanding these processes allows for a more informed and regenerative approach to aesthetic treatment.

Why Collagen, Bone Density, and Fat Compartments Diminish with Age

Ageing is a biologically programmed, multifactorial process influenced by genetics, hormonal changes, environmental exposure, and cellular decline.

  • Collagen loss: From our mid-20s onward, collagen production declines by approximately 1% per year. After menopause, this decline accelerates significantly due to reduced oestrogen levels.
  • Bone density reduction: With age, there is gradual resorption of bone, particularly in the midface, orbital rim, and jawline. This process alters the structural scaffolding that supports the overlying soft tissue.
  • Adipose tissue changes: Facial fat compartments shrink, descend, and redistribute over time. Rather than simply “losing volume,” we experience selective atrophy in some areas and relative hypertrophy in others.
